Structure is only one layer

Organization charts show formal reporting relationships. They do not fully explain why a role exists, what outcomes it owns, which decisions it can make, or how it must work across boundaries.

When these questions remain open, people recreate the old organization inside the new structure.

Define accountability in practical terms

Role clarity improves when outcomes, decision rights, interfaces, and measures are discussed together. The aim is not exhaustive documentation; it is enough precision for people to act.

Reinforce through leadership routines

Leaders embed the design through the meetings they run, the choices they escalate, and the behaviour they reward. Governance must reflect the intended accountabilities.
